You might want to try different login settings; it looks like your ISP is not getting the information required to login.

I had problems like yours when initially trying to set up my dial-up connection. It is working fine for me now. I was, of course, too stupid to keep track of the different things I tried thus have NO idea of which tweak actually fixed the problem.
