[hcflinux] Cannot determine ethernet address for proxy ARP

I just installed the driver by using the installer 
(http://www.linuxant.com/drivers/hcf/downloads-installer.php).

For what it is worth, my modem is TeleWell TW-5614SI
pci v.92(h).

When I try to connect to the internet, I can not
transfer data using the Pulses dial type. Look below
for output of  plog command:
