Hi,
your modem with PCI ID 1002:4378 is actually an AC97 bus to which a
modem is connected. This modem could be provided by various
manufacturers and only Conexant is supported by our HSF modem driver.
The source of the problem is likely:

We have not yet tester our products on Rawhide but a few users have
reported the same problem on SuSE 10.0. The problem was that the
'modprobe' tool was using the '/etc/modules.conf' configuration file
instead of using the '/etc/modprobe.conf' file (or a similar file). If
could could try to remove the '/etc/modules.conf' file from your system
and then run the following commands in a root shell:

depmod
hsfconfig --info
---
Finally, the following error messages:

hsfmc97ati: Unknown symbol pm_register
hsfmc97ati: Unknown symbol pm_unregister

is new to us. It is possible that these functions were removed in a
kernel later than 2.6.15-rc1 (even if the kernel is named
2.6.14-1.1740_FC5, is is quite possible that the core of the kernel is
based on 2.6.15-rcX). Our developers are currently taking a look at this
problem and it is quite possible that it will be fixed in the next
release of the HSF modem driver.
Finally, please note that even if you fix all the problems mentioned
above, your system is likely to crash when inserting the HSF modem
driver modules into the kernel. This is a known problem with version
7.18.00.07full (x86_64 only) of the HSF modem driver and a kernel 2.6.11
or later. This problem will be fixed in the next release of the driver
and the only known workaround is to use a 2.6.10 or previous kernel.
